Medication Review Appointments

To Book an Appointment for a thorough Medication Review with one of our Pharmacists:

1. Choose the Date and Time
2. Then below the Calendar fill out your Email Address, Name, Phone and any Personal Message
3. Review the Booking Overview and click Submit!
4. To Change Any Info: simply redo the info and make sure the changes show in the Booking Overview below
   and click Submit.
